How to File a Request

You can file a Right-to-Know request in four ways:

• Fax: Download the RTK Request Form, fill it out, and fax it to 412-751-6002
• Electronic Mail: Please fill out the online email form below.
• In Person: Visit the Municipal Building Office. Hours are Monday through Friday, from 8:00 A.M. to 4:30 P.M. except for holidays.
• U.S. Mail: Download the RTK Request Form, fill it out, and mail it to:

522 Rock Run Road, Elizabeth, PA 15037

Always retain a copy for your personal file.

Upon denial, a copy of the RTK request would be necessary if you should file an appeal to the:
Pennsylvania Office of Open Records

*If you do not have a copy of your request, your appeal will be dismissed as insufficient.
